This page contains a complete and current list of all providers and health care facilities whose services are no longer covered by the Plan.

Any of the health care service providers working under the direction or supervision of Dr. Lawrence Saks, M.D.,
Any of the health care facilities in which Dr. Lawrence Saks, M.D. has an ownership interest, including, but not limited to, Madison Park Surgery Affiliates and Reconstructive Surgery Affiliates
Please take notice that the Trustees of the ILWU-PMA Welfare Plan, who have sole authority to administer the Plan, have unanimously determined that the Plan will not reimburse you or otherwise pay any claims for services or supplies provided to you by the above providers.
